Abstract Background Our goal was to discuss the treatment for rupture of contralateral mainstem bronchus during uniportal video-assisted thoracoscopy surgery (uniportal VATS) lobectomy. Case presentation We analyzed clinical data 3 cases VATS. Surgical repair performed immediately under an VATS operation, as a result, bronchial all were repaired successfully, and we continued complete lobectomy systemic lymph node dissection. Reexamination after 1 week, no fistula found in trachea bronchi through fiberoptic bronchoscopy. The time range indwelling chest tube is 6–9 days, hospital stay 8–10 days. No abnormality observed on radiography when patients returned month operation second reexamination. Conclusions Instant surgical recommended thoracic surgery. It safe feasible tear with
